    Specialist Support with the NDIS

    We offer Level 3 NDIS Specialised Support Coordination

    Our Specialist Support Coordinators work with you and your support networks to coordinate complex NDIS plans.

    How Can We Help?

    Our Specialist Support Coordinators help people living with a disability that have additional complexities, this may involve government departments such as health, housing, justice and mental health systems.

    We work with you to understand any barriers to access and maintain supports to help achieve your goals.

    Our Specialist Support Coordinators aim to empower participants and their support networks. We understand that each person’s situation is different, and we provide an individual and holistic service.

    If you’re eligible, the National Disability Insurance Agency (NDIA) may fund Specialist Support Coordination in your NDIS plan. Some factors may include:

    • Housing, accessibility and displacement
    • Complex health comorbidities
    • Lack of access to supports eg rural area, communication barriers, social isolation
    • Involvement with other government departments (Justice System Corrective Services, Health, Housing)
    • Currently in a crisis or high risk situation
    • Other barriers to support

    What kind Support Coordination do I have?

    The NDIA have three levels of Support Coordination which allows you different support needs. Read below to find out the difference:

    Level 1 Support Coordination – Support to help a participant understand their plan, connect with NDIS providers and community, mainstream and other supports to get the most out of their NDIS plan.

    Level 2 Support Coordination – Support to put in place a mix of supports to increase a participant’s capacity to maintain relationships, manage tasks, live more independently and be included in their community.

    Level 3 Specialist Support Coordination – A higher level of support for participants whose situation is more complex. Our Specialist Support Coordinators can assist in designing a personalised support plan that can be implemented to address complex barriers to ensure consistent delivery of service.

    If the level of support coordination funding is not described in the plan, participants can choose what level of support coordination is most appropriate for their needs and how it’s delivered.

    This could involve working with a Specialised Support Coordinator and/or a Psychosocial Recovery Coach to implement your support plan.

    Yes! We can assist NDIS participants within the justice system, and work with case managers and other contacts to identify and ensure the right supports are in place to assist in the process to reintegrate into the community.

    Specialised Support Coordinators is an NDIS support and can be funded through your plan.

    The NDIS provides information for all NDIS supports and the pricing, this us updated periodically. Please view the NDIS Pricing arrangements to familiarise yourself with their approach.

    The initial meeting is an opportunity form a mutual understanding of your situation and what matters to you. We may discuss agreements, responsibilities and your rights to ensure transparency and flexibility.

    This can be how often and how we communicate regarding more complex concerns such as consent and risk management. We may work together on a individualised support plan to help in identifying immediate priorities, needs, preferences, gaps in support and how to can address and manage them.

    Our Specialised Support Coordinators help individuals to understand their NDIS plans, connect them with providers and manage services to assist individuals to achieve their personal goals.

    Specialised Support Coordinators are not Support Workers.
